NOTICE OF PUBLIC HEARING Two public hearings will be held at the Community Heart & Vascular Hospital in the Board Room adjacent to the Main Lobby, located at 8075 N. Shadeland Avenue, Indianapolis, IN 46250, on Tuesday, December 16, 2025: 1st Session from 3:30 PM to 4:00 PM and the 2nd Session from 4:00 PM to 4:30 PM. These public hearings will include an opportunity for representatives from Community Health Network to present information regarding the project and answer any questions or concerns of the public. About the Project: The Community Heart & Vascular Hospital is investing approximately $97 million to expand and renovate the facility. The project is being completed in multiple phases with the final phase being completed in mid-2030. During the project, the Community Heart & Vascular Hospital will continue normal operations and existing practice areas and patient care will be minimally impacted. This investment will bring enhanced and expanded services to patients at the Community Hospital North Campus. Project Overview: The program consists of two single-story expansions totaling 30,200 square feet and extensive renovations of the Community Heart & Vascular Hospital.
