DOB: 08/08/2021                                            SUMMONS AND NOTICE OF HEARING          A Child(ren).   TO: Child’s possible biological father You are hereby commanded to appear before this Court in the Courtroom, Boone County Court House, Boone, Iowa, on June 1, 2026, at 9:00 AM. You shall bring the child named above with you at that time. You shall bring the child(ren) named above with you at that time. If you fail without reasonable cause to appear or to bring the child(ren), you may be proceeded against for contempt of court or the Court may issue an order for your arrest, or both your arrest and the taking into the custody of the child(ren).   YOU ARE HEREBY NOTIFIED OF THE FOLLOWING:   1. There is presently on file in this court a verified petition alleging that the child(ren) named above is a child(ren) who is alleged to be a child(ren) in need of assistance: a copy of the petition is attached. An adjudicatory hearing on the merits of the petition is set for the time and place shown above. 2. Said child(ren) has/have the right to be represented by counsel (a) at a removal hearing, (b) at a shelter care hearing, (c) at an adjudicatory hearing, (d) at a dispositional hearing, and (e) at hearings to review and modify a dispositional order. 3. A parent, guardian, or custodian has the right to be represented by counsel at all hearings and proceeding subsequent to the filing of a petition. If you desire counsel but are financially unable to employ counsel, the Court will appoint free counsel for you if you make application to the Clerk of Court. You should do this immediately. 4. A hearing or proceedings may take place without your presence if you fail to appear after reasonable notification. 5. Upon your request, the clerk shall issue subpoenas requiring the attendance and testimony of witnesses and the production of papers at the hearing. 6. If you wish to state your views to the Court, you must appear or the Court may rule against you in your absence.
