NOTICE OF DISSOLUTION OF DIVINITY AESTHETICS AND ANESTHESIA, S.C. NOTICE IS HEREBY GIVEN in accordance with Wis. Stat. § 180.1407 that: (1) Divinity Aesthetics and Anesthesia, S.C., a Wisconsin service corporation, has dissolved effective December 31, 2025; (2) all persons with claims, whether known or unknown, against Divinity Aesthetics and Anesthesia, S.C. or its directors, officers, or shareholders, in their capacities as such, must present such claims in writing via first class mail, postage prepaid, to Attorney Lora Zimmer at McCarty Law LLP, 2401 East Enterprise Ave., Appleton, WI 54913; (3) each claim must describe in detail the nature and amount of the claim or an estimate of such amount, the date upon which the claim arose, and the facts and circumstances giving rise to the claim; (4) each claim must be accompanied by documentation supporting or substantiating the claim; and (5) any alleged claims against Divinity Aesthetics and Anesthesia, S.C. or its directors, officers, or shareholders will be barred unless a proceeding to enforce the claim is commenced within 2 years after the publication of this notice.
