Important Notice on PacificSource D-SNP Plan Termination January 31, 2026 (Springfield, OR) Dec. 23, 2025 – PacificSource Community Health Plans is making changes to its Dual Eligible Special Needs Plan (D-SNP) operations for 2026 to ensure we are delivering the best possible outcomes for the people we serve. After careful evaluation, and in alignment with regulatory guidance and our planned Medicaid exits, we will reduce our D-SNP service area by exiting Lane County effective January 31, 2026. D-SNP is designed to provide highly coordinated care between Medicare and Medicaid. Because PacificSource will no longer administer Medicaid in Lane County, we would not be able to maintain the level of seamless, integrated care that D-SNP is intended to deliver. This change ensures affected members can transition to coverage options that continue to provide coordinated care. If members do not choose another Medicare health or prescription drug plan by January 31, 2026, they will automatically be placed in Original Medicare starting February 1, 2026. Medicare will also assign members to a separate prescription drug plan. Members can choose other coverage options, including another Medicare Advantage plan, an integrated Dual Eligible Special Needs Plan (D-SNP) or original Medicare with a stand-alone prescription drug plan. Members have a Special Enrollment Period through March 31, 2026, to make changes.
