NOTIFICATION OF INTENT TO APPLY FOR A CERTIFICATE OF NEED This is to provide official notice to the Health Facilities Commission and all interested parties, in accordance with T.C.A. §68-11-1601 et seq., and the Rules of the Health Facilities Commission, that Stern Cardiovascular Surgery Center, LLC, a/an Ambulatory Surgical Treatment Center (ASTC) – Single Specialty owned by The Stern Cardiovascular Center, P.A. with an ownership type of Professional Association and to be managed by Atria Health Services of the South, LLC intends to file an application for a Certificate of Need for the establishment of a single specialty, cardiovascular ambulatory surgical treatment center and the initiation of diagnostic and therapeutic cardiovascular catheterization and diagnostic and therapeutic electrophysiology procedures that the Centers for Medicare & Medicaid Services has approved for performance in the ASTC setting.
