THE STATE OF OKLAHOMA TO:
Christopher Avery Warren, Sr. and Theresa Bridges
YOU ARE HEREBY NOTIFIED the State of Oklahoma, through Jeff S. Jones, Prosecuting Attorney in and for the Tenth Judicial District of Oklahoma, has filed a petition alleging the above-named child(ren) as deprived and the same has been set for Initial Appearance on Petition hearing on the 2nd day of September, 2026 at 10:00 AM in the Osage County Courthouse, 600 Grandview, Pawhuska, Oklahoma.
FURTHER, the above-named parent(s) must appear before the District Court in and for Osage County, Juvenile Division, thereof, in the courtroom in Pawhuska, Oklahoma, before Judge Stuart Tate on the 2nd day of September, 2026 at 10:00 AM, when and where the Court will proceed to hear the merits of said petition and grant such relief or make such orders as the Court deems necessary and proper in these premises.
You are advised that the parents are entitled to be represented by an attorney at each and every stage of these proceedings. If a state of indigence exists, such parties are entitled to a Court-appointed attorney at public expense.
FURTHER, that in the event the parent(s), Christopher Avery Warren, Sr. and Theresa Bridges, fail to appear as summoned herein, the Court will proceed to grant such relief and make such orders as the Court deems necessary and proper in said cause, as supported by the findings on the merits of said cause of action.
FAILURE TO APPEAR AT THIS HEARING CONSTITUTES CONSENT TO THE ADJUDICATION OF THIS CHILD (OR THESE CHILDREN) AS DEPRIVED CHILDREN AND MAY ULTIMATELY RESULT IN LOSS OF CUSTODY OF THIS CHILD OR THE TERMINATION OF PARENTAL RIGHTS TO CHILD(REN).
