DONALDSONVILLE HOUSING AUTHORITY REQUEST STATEMENT OF QUALIFICATIONS GRANT CONSULTANT FOR THE AGENCY AND CAPITAL FUND PLAN The Housing Authority of the Donaldsonville, is requesting Statements of Qualifications from qualified firms to prepare its Annual and 5-Year Agency and Capital Fund Plans, and when funded, provide Grant Monitoring and Reporting Services for the Housing Authority. All services must be in accordance with the latest HUD rules applicable to the program, and all applicable Federal, State, and Local requirements, with certifications normally associated with Federally funded projects. The program contains management improvements, construction projects, and the purchase of equipment. A. SERVICES REQUIRED The services required are to provide application, and when funded, grant monitoring and reporting services for the Housing Authority Annual Plan and its CFP Annual Plan, generally as follows: 1. Update current 5-Year PHA Agency Plan and Annual Plans with required documentation to remain in compliance with published HUD regulations. 2. Prepare next 5-Year PHA Agency Plan with Annual Plan and subsequent Annual Plan with required documentation in accordance with published HUD regulations. 3. Prepare 5-Year CFP Action Plans and CFP Annual Statements when annual allocations are made with required documentation for CFP Program in accordance with published HUD regulations. 4. Assist as may be needed with the preparation for Board consideration of all budget revisions and review materials. 5. Assist to establish controls as may be needed to manage the financial aspects of the programs. 6. When funded, prepare LOCCS Payment Vouchers, periodic updates of program amounts obligated and expended as required by the CFP program, and all semi-annual and final Performance and Evaluation Reports required for the program. 7. Assist with program inquiries from HUD and other agencies. 8. Prepare monitoring reports, evaluation documents, certificates of completion, and closeout documents as may be required for the CFP program. 9. Conduct public hearings, attend meetings, prepare documents, etc., as may be required for the program. B. STATEMENTS OF QUALIFICATIONS 1. Basic information about firm, principals, and personnel to be assigned to this project. 2. Information about the firm's experience with The PHA Agency Plan, the CFP Program, other similar HUD programs, and any special qualifications. 3. Evidence of knowledge and training regarding The PHA Agency Plan, the CFP Program, other similar HUD programs, and relative experience. 4. Evidence of intent and capability to perform services in a timely manner. 5. Firm's knowledge of the special needs and circumstances of the Housing Authority. 6. Other factors: a) knowledge of Davis Bacon, HUD 504, and lead base paint requirements b) Previous experience working with housing authorities DHA operates 152 public housing units and currently has two Capital Fund Programs. DHA may reject any and all Statement of Qualifications, re-advertise, postpone or cancel this request at its discretion. DHA reserves right to waive any formalities. All Statements of Qualifications will be evaluated and a contract will be awarded to the contractor deemed most advantageous to DHA.
