Dr. Jason W. Thackeray - Retiring Effective December 31, 2025, Dr. Jason W. Thackeray will be retiring from Orthopaedic Associates. All patient medical records will remain with Orthopaedic Associates. Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only with your permission. If you plan to continue with this office, there's nothing you need to do. If you choose to transfer your care to a different practice or physician and need to have your records picked up, mailed, or transferred, you will need to sign the appropriate Authorization for Release form. This form is available upon request in person at 1034 Mar Walt Drive, Fort Walton Beach, FL, 32547, or by calling our office at 850-863-2153. You should return the signed request to our office as soon as possible so we can transfer your records to your new doctor. Until then, your records will remain on file at Orthopaedic Associates.
