Notice of Suit The State of Indiana to the Defendant above named, and any other person who may be concerned. You are notified that you have been sued in the Court above named. The nature of the suit against you is: On or about March 5, 2024, Third-Party/Crossclaim Plaintiff was operating a motor vehicle on I-65southbound in Indianapolis, Marion County, Indiana. The Third-Party/Crossclaim Defendant, First Class Steamer Carpet Cleaner LLC, left an abandoned vehicle on I-65southbound in Indianapolis, Marion County, Indiana, and in violation of the common law and the statutes of Indiana, was negligent in the abandonment of said motor vehicle, causing Third-Party/Crossclaim Plaintiff's vehicle to be struck by another vehicle. The summons by publication is specifically directed to the following named Third-Party/Crossclaim defendant whose addresses are: First Class Steamer Carpet Cleaner LLC c/o Daniel Montalvo, Registered Agent 5602 Elwood Ave. Indianapolis, IN 46203 First Class Steamer Carpet Cleaner LLC c/o Daniel Montalvo, Registered Agent 5602 Elwood Ave., Apt. 122 Indianapolis, IN 46203 First Class Steamer Carpet Cleaner, LLC c/o Daniel Montalvo, Registered Agent 2302 N. Lynhurst Dr., Apt. C Indianapolis, IN 46224 And to the following Third-Party/Crossclaim defendant, First Class Steamer Carpet Cleaner LLC, whose whereabouts are unknown, In addition to the above-named Third-Party/Crossclaim defendant being served by this summons there may be other Third-Party/Crossclaim defendants who have an interest in this lawsuit. If you have a claim for relief against Third-Party/Crossclaim plaintiff arising from the same transaction or occurrence, you must assert it in your written answer. You must answer the Complaint in writing, by you or your attorney, on or before thirty days after the Third Notice of this Suit, and if you fail to do so a judgment will be entered against you for what the plaintiff has demanded.
