Patients of Purath Headache and Neurology, LLC in Greenfield, WI Medical Records To all patients of Purath Headache and Neurology, a wholly owned subsidiary of Cephalgia Services Management Group, an Illinois LLC. Purath Headache and Neurology will no longer be seeing patients and will be permanently closing its doors effective December 31, 2025. Effective January 1, 2026, Dr. Traci Purath will be seeing patients at Center for Neurological Disorders, SC in Greenfield, WI. If, as a patient, you have not already requested your medical records from Purath Headache and Neurology and would like them please email your medical records request to tia.wierman@diamondheadache.com to complete this process. Please include your name, date of birth, and mailing address for your records in your correspondence. Alternatively, you may submit a records request in person by visiting Purath Headache and Neurology at 7701 West Barnard Avenue in Greenfield, WI 53220; the office hours are Monday-Friday from 8:30am to 4:00pm. In person requests must be received prior to December 31, 2025. Once a valid request has been received your records will be mailed to the address provided in the request. Please note that any and all records for which we have not received a valid transfer request by January 26, 2026 will be permanently destroyed and no longer accessible on January 30, 2026.
