NOTICE OF ACTIVITY AND USE LIMITATION [120 Rear] GILBOA STREET, DOUGLAS MA RELEASE TRACKING NUMBER 2-0022444 A release of oil and/or hazardous materials has occurred at this location, which is a disposal site as defined by M.G.L. c. 21E, § 2 and the Massachusetts Contingency Plan, 310 CMR 40.0000. On May 13, 2026, Gilboa Properties LLC recorded with the Worcester South District Registry of Deeds a Notice of Activity and Use Limitation on the disposal site, pursuant to 310 CMR 40.1070 through 40.1080. The Notice of Activity and Use Limitation will limit the following site activities and uses on the above property: ¨ Use as a residence, children's school, child daycare facility, or children's recreational area; ¨ Farming, vegetable or fruit gardening, or other agricultural use of soils at or from the Property to grow produce for human consumption; and ¨ Activities or uses (other than emergency repair and/or maintenance of utilities) that could involve direct contact with potentially contaminated surficial or subsurface soil, unless conducted in accordance with a written Health & Safety Plan approved by a Licensed Site Professional (LSP) and a Soil Management Plan approved by an LSP.
