Notice of Dissolution Iowa Code Section 504.1407 To all persons interested in and claimants of Central Iowa Housing Foundation, Inc. an Iowa corporation, f/k/a Polk County Health Services Foundation, has filed a Statement of Dissolution effective December 31, 2025. All persons with claims against Central Iowa Housing Foundation, Inc. must present them as follows: 1. The claim must include the name of the claimant, the address of the claimant, the telephone number of the claimant, the amount claimed, the date the claim was incurred, and the detailed basis for said claim. 2. The claim must be accompanied by all written evidence of said claim, including, but not limited to, invoices, leases, notes and documents evidencing security interests. 3. Any other evidence supporting a claim against Central Iowa Housing Foundation, Inc. an Iowa corporation, f/k/a Polk County Health Services Foundation. Claims must be addressed and mailed postage prepaid to Central Iowa Housing Foundation, Inc., Attn. Justin L. Sullivan, Esq, Whitfield & Eddy, PLC, 699 Walnut St. Suite 2000, Des Moines, IA 50309. NOTWITHSTANDING FILING OF THE CLAIM, ANY CLAIM AGAINST CENTRAL IOWA HOUSING FOUNDATION, INC.
