Rutherford Street, Athens, Clarke County, Georgia 30606, has been delivered to the Georgia Secretary of State for filing in accordance with Section 14-11-606 of the Georgia Limited Liability Company Act. Any persons with claims against Harlan J. Bruner, M.D., LLC may submit such claims to: V.
