Notice of Intent to Transfer Medical Records Pursuant to South Carolina Code of Laws §44-115-130, The Children's Medical Center, P.A. (the "Practice") hereby provides notice of its intent to transfer medical records of its patients to Tribe513, P.A., which operates Parkside Pediatrics, as part of an Asset Purchase Agreement, effective July 20, 2026. After that date, the PRACTICE PHYSICIANS WILL CONTINUE TO PRACTICE AND PROVIDE PROFESSIONAL MEDICAL SERVICES TO PATIENTS, WITHOUT INTERRUPTION, AT THE PRACTICE'S CURRENT PRACTICE LOCATIONS, as employees of Tribe513, P.A. Patients of the Practice may retrieve their records from the Practice if they prefer that their records not be included in the transfer. July 15, 16, 17 2026 LGSC0555444
