NOTICE TO CREDITORS OF MICHELLE ELLSWORTH Denise Rall whose address is c/o ProvenLaw, PLLC, 216 W. St. George Blvd., Ste. 200 St. George, UT 84770, is the Personal Representative of the Estate of Michelle Ellsworth, also known as Michelle Arline Ellsworth, and Denise Rall is also the Trustee of the Michelle Ellsworth Trust, dated October 18, 2022 (the "Trust"). Michelle Ellsworth, also known as Michelle Arline Ellsworth, Grantor of the Trust, died on May 4, 2026 Utah Code Annotated § 75-3-801, and Utah Code Annotated § 75B-2-508 provide that any creditor having a claim against the Estate of Michelle Ellsworth, also known as Michelle Arline Ellsworth, or the Trust, must file their claim with Denise Rall, Personal Representative/Trustee, or Kristin K. Woods, attorney for Denise Rall, Personal Representative and Trustee, at 216 W.
