NOTICE TO CREDITORS OF THE Lesley & William Cook Trust, dated February 19, 2025 Lesley Cook, whose address is c/o ProvenLaw, PLLC, 216 W. St. George Blvd., Ste. 200 St. George, UT 84770, is the Trustee of the Lesley & William Cook Trust, dated February 19, 2025, (the "Trust"). William Cook, also known as William Allen Cook, Grantor of the Lesley & William Cook Trust, died on March 10, 2026. Utah Code Ann. § 75B-2-508 and 75-3-801, provide that any creditor having a claim against the Trust or the Estate of Wiliam Cook must file their claim with Lesley Cook, Trustee, or Lindsay M.
