VERIFIED PETITION TO ESTABLISH PATERNITY AND CUSTODY
COMES NOW, Petitioner, Sarah Mason (hereinafter "Mother"), by Counsel, Justin C. Smith, and heretofore files her Verified Petition to Establish Paternity and Custody and in support thereof states as follows:
That Respondent is the biological Father of Carter Liggins (hereinafter "Minor Child").That the Minor Child was born out of wedlock on June 2, 2011.That Father has properly signed the paternity affidavit and is named on the child's birth certificate.That Mother currently resides at 6240 W. Ralston Road, Indianapolis, IN 46221.That Father's current residence is unknown to Mother.That Mother respectfully requests the Court enter an order establishing paternity for the minor child with Father, Desi Liggins, being the biological Father of the minor child.
That Mother respectfully requests this Court enter an order establishing custody as to Mother as the sole legal and primary custodian of the minor child.That Mother respectfully requests this Court set a hearing on this matter at the Court's earliest convenience.
WHEREFORE, the Petitioner respectfully requests this Court grant her Verified Petition to Establish Paternity and Custody and for all other relief just and proper in the premises.

You are hereby notified that you have been sued by the Petitioner in a Paternity action in the Court indicated above. If this summons is accompanied by an Order to Appear, you must appear in Court on the date and time stated in the Order to Appear. If you do not appear, evidence may be heard in your absence and a determination made by the Court. If a Temporary Restraining Order is attached, it is effective immediately upon your receipt or knowledge of the Order. If you wish to retain an attorney to represent you in this matter, it is advisable to do so before the date stated in the Order to Appear.If you take no action in this case after receipt of this summons, the Court can grant a Dissolution of Marriage or make a determination regarding any of the following: paternity, child custody, child support, maintenance, visitation, property division (real or personal) and any other distribution of assets and debts.
