TRANSFER OF OWNERSHIP Please be advised that effective December 12, 2025, Pain Relief Institute, LLC acquired First Coast Medical Center, Inc. We are pleased to announce that Dr. Pedro G Mascaro is the new Medical Director and Owner of Pain Relief Institute, LLC. Additionally, his Specialty is Interventional Pain Medicine which allows our practice to provide comprehensive specialty services to our patients. Dr. Mascaro and the existing clinical team, whom you already know, are available to provide full continuity of your care, without interruption, at all of our existing office locations. Pain Relief Institute, LLC is the new custodian of all patient medical records. In accordance with Florida law, your medical records remain securely maintained via our Electronic Medical Records system. You have the right to: 1. Continue your treatment with your respective Chiropractic Providers as well as initiate care with Dr. Mascaro as a Pain Medicine Specialist. 2. Request a copy of your medical records. 3.
