PUBLIC NOTICE OF HOSPICE PROVIDER CLOSURE
Notice is hereby given that Empyrean Hospice of GA, a hospice provider licensed in the State of Georgia and Medicare-certified to provide hospice services, will cease hospice operations effective July 5, 2026.
As of the effective closure date, Empyrean Hospice of GA will no longer provide hospice services to patients in Banks, Barrow, Clarke, Dawson, Elbert, Fannin, Franklin, Gilmer, Greene, Gwinnett, Habersham, Hall, Hart, Jackson, Lumpkin, Madison, Morgan, Newton, Oconee, Oglethorpe, Rabun, Rockdale, Stephens, Towns, Union, Walton, White, and/or Wilkes counties.
Current patients, guardians, authorized representatives, families, referral sources, and care partners are being notified directly and assisted with transition of care to another hospice provider of the patient's choice. Hospice services will continue through the effective closure date or until the patient's care is transferred to another hospice provider, whichever occurs first.
Medical records will be maintained by Empyrean Hospice – Medical Record Division. Requests for medical records may be directed to:
