Notice to Wellmark Health Plan of Iowa, Inc. Enrollees Wellmark Health Plan of Iowa, Inc. members (referred to as "enrollees") may submit application forms to be considered for nomination as enrollee directors on the Company's board of directors. Enrollee directors must be currently enrolled in Wellmark Health Plan of Iowa, Inc. health plan coverage and must be adult residents of the state of Iowa. Enrollee directors cannot be practicing health care providers or employed by a health care provider and cannot be financially interested in any competitive organization. Completion of an application form and a conflict of interest form is required and completed forms with a signed certification must be filed with the Secretary of the Company no later than May 8, 2026. Nominations for an enrollee director should include the nominee's qualifications. Nominees, if any, will be determined by the Board of Directors and elected by the Shareholder.
