Notice of Demand for Issuance of Duplicate Stock Certificate Issued by North Poudre Irrigation Company A "Statement of Loss Under Oath" has been filed with The North Poudre Irrigation Company with its principal office at 3729 Cleveland, Wellington, CO 80549 and mailing address of P.O. Box 100, Wellington, CO 80549 pertaining to one (1) lost, mislaid, or destroyed contract certificate ("Certificate"). Certificate No. 12356 represents one (1) share of the North Poudre Irrigation Company. Said Certificate was issued by The North Poudre Irrigation Company. The North Poudre Irrigation Company will issue, on or after June 12th, 2026, a duplicate replacement Certificate to the registered owner unless a contrary claim is filed with The North Poudre Irrigation Company prior to said date. In accordance with the terms of C.R.S. 7-42-114 through 7-42-117, the "Statement of Loss Under Oath" states, among other items, as follows: 1. That such Certificate has been lost, mislaid or destroyed and that all assessments levied by The North Poudre Irrigation Company against such water right represented by this Certificate have been paid prior to the date of this Notice of Demand. 2. Lost Certificate No. 12356 was issued on January 1st, 2017, to Karen Surdal. 3. The Certificate owner, Karen Surdal, agrees to hold harmless and indemnify The North Poudre Irrigation Company, its officers, directors and employees for all losses, damages and attorney fees, if any, for any claims made against The North Poudre Irrigation Company as a result of issuing such duplicate water right Certificate. 4. Notice is given herewith that one (1) Certificate for one (1) share will be issued by The North Poudre Irrigation Company to Karen Surdal, on or after June 12th, 2026, unless a contrary claim is filed with The North Poudre Irrigation Company prior to said date. 5. A copy of the claim should also be sent to general counsel for The North Poudre Irrigation Company, Anderson and Hinze Law, 355 Eastman Park Drive, Suite 200, Windsor, Colorado 80550.
