NOTICE OF DISSOLUTION NORTHEAST GI RESEARCH DIVISION, LLC NOTICE IS HEREBY GIVEN that Northeast GI Research Division, LLC, a Tennessee limited liability company (the "Company"), is dissolved. Articles of Termination for the Company were filed on January 26, 2026, with the office of the Tennessee Secretary of State. All creditors and other persons with claims against the Company are hereby notified to file their claims against the Company. Claims must be in writing. Claimants must state the amount of their claim, state the nature of the claim, provide the name and address of the claimant, and be signed. Claims must be mailed to the Company at Northeast GI Research Division, LLC, C/O ObjectiveHealth, Inc., 341 Cool Springs Boulevard, Suite 450, Franklin, Tennessee 37067. Unless earlier barred, claims against the Company will be barred unless a proceeding to enforce the claim is commenced within two years after the publication of this Notice.
