NOTICE TO CREDITORS OF Jo Ann Harper Family Trust, dated June 7, 2017 Gary K. Lamph, whose address is c/o ProvenLaw, PLLC, 216 W. St. George Blvd., Ste. 200 St. George, UT 84770, is the Trustee of the Jo Ann Harper Family Trust, dated June 7, 2017 (the "Trust"). Jo Ann Harper, Grantor of the Trust, died on January 28, 2026. Utah Code Ann. § 75B-2-508 and 75-3-801, provide that any creditor having a claim against the Trust or the Estate of Jo Ann Harper must file their claim with Gary K. Lamph, Trustee, or Lindsay M.
