AND INSURANCE COMPANY OF THE WEST NOTICE OF HEARING TO DETERMINE BENEFICIARIES AND HEIRS Notice is hereby given to all persons interested in the workers' compensation claim filed by Cheryl A. Taylor against Kimball Ready Mix, Inc. and Insurance Company of the West based on the death of Tony T. Taylor from an alleged work-related injury on or about October 28, 2025 in or near Fairview, Oklahoma. The hearing is scheduled for hearing before Administrative Law Judge John Blodgett at 9:00 AM on July 13, 2026 at the Oklahoma City location of the Oklahoma Workers' Compensation Commission located at 1915 N. Stiles Ave., Oklahom City, OK. A hearing will be conducted to determine any heirs, survivors, or other persons who are eligible for workers' compensation benefits based on the death of Tony T. Taylor. All persons interested in the claim or who have any interest in the estate of Tony T. Taylor may appear and put forth evidence to show the basis for entitlement to benefits based on the death of Tony T. Taylor. Failure to appear for the hearing described herein may result in an Order awarding benefits limited to only those persons who appear and present evidence to establish their claim.
