PREQUALIFICATION PROPOSALS
Sealed proposals will be received on November 20, 2025 at 2:00 p.m. by the City of Jackson Health Care Authority at 220 Hospital Drive, Jackson, Alabama 36545 at the Main Conference Room at which time and place they will be publicly opened and read for:
JACKSON MEDICAL CENTER IMPROVEMENTS
for The City of Jackson Health Care Authority

The Work of the project includes, but is not limited to: Site work and utility upgrades, approximately 53,000 square feet of roof replacement to the existing Medical Center, construction of a new Medical Clinic approximately 6,100 square feet and construction of a new Emergency Department approximately 5,900 square feet. The full scope of work is included in the Prequalification Package.
Only General Contractors that are prequalified may attend the mandatory pre-bid meeting and be allowed to submit a bid proposal for this project. Prequalification packages shall be delivered by hand or mail to Goodwyn Mills Cawood, LLC; Attn: Doris Furr, 11 North Water Street, Suite 19290, Mobile, AL 36602 by 2:00pm CST September 24, 2025.
All prospective bidders shall submit a written proposal using the American Institute of Architects (AIA) Document A305, “Contractor’s Qualification Statement,” latest edition, which may be purchased from an AIA documents vendor.
No proposals will be considered unless the bidder, whether resident or non-resident of Alabama, is properly licensed and qualified to submit a proposal for this construction in accordance with all applicable laws of the State of Alabama. This shall include evidence of holding a current license from the State Licensing Board for General Contractors, Montgomery, Alabama, as required by Chapter 8 of Title 34, of the Code of Alabama, 1975. In addition, non-residents of the State, if a corporation, shall show evidence of having qualified with the Secretary of State to do business in the State of Alabama.
Prior to the award of a competitively bid contract to a contractor having one or more employees in the state of Alabama, Alabama law requires that the contractor provide the county proof of enrolment in E-Verify (see www.uscis.gov/everify).
A cashier’s check or bid bond payable to The City of Jackson Health Care Authority in an amount not less than five (5) percent of the amount of the bid, but in no event more than $10,000, must accompany the bidder’s proposal. Performance and statutory Labor and Material Payment Bonds, insurance in compliance with requirements, and verification of E-Verify enrollment will be required at the signing of the Contract.
Bid Documents, Drawings and Specifications will be available beginning October 23, 2025 and may be examined during normal business hours at the Office of the Architect; Digital Plan Rooms of Dodge, Construction Data Fax, and Construct Connect.
General Contractors must procure plans and specifications from the Architect upon payment of a deposit of Twenty dollars ($20.00) for a one-time administrative fee for digital/file sharing access or Two Hundred dollars ($200.00) (printed) per set. Contractors are encouraged to use the digital plans. Refunds will be issued for printed sets only issued by the Architect to each general contractor bidder on the first two (2) sets issued submitting a bonafide bid, upon return of documents in good and reusable condition within ten (10) days of bid date. Additional sets for General Contractors, and sets for subs and vendors, may be obtained with the same deposit, which will be refunded as above, less cost of printing, reproduction, handling and distribution, which is estimated to be the same as the deposit amount.

Bids must be submitted on proposal forms furnished by the Architect or copies thereof. All bidders bidding in amounts exceeding that established by the State Licensing Board for General Contractors must be licensed under the Provision of Title 34, Chapter 8, Code of Alabama, 1975, as amended, and must show such evidence of license before bidding or bid will not be received or considered by Architect or Owner; The bidder shall show such evidence by clearly displaying their current license number on the outside of the sealed envelope in which the proposal is delivered; Bidder must also include their current license number on the Proposal Form. No bid may be withdrawn after the scheduled closing time for receipt of bids for a period of SIXTY (60) days.
A PRE-BID CONFERENCE will be held at the City of Jackson Health Care Authority at 220 Hospital Drive, Jackson, Alabama 36545 at the Main Conference Room, on October 30, 2025 at 2:00 p.m. CST, for the purpose of reviewing the project, answering Bidder’s questions, and touring the work areas. Attendance at the Pre-Bid Conference is mandatory for all General Contractor Bidders intending to submit a Proposal.

The Owner reserves the right to reject any or all proposals and to waive technical errors if, in their judgment, the best interests of the Owner will thereby be promoted.
