REQUEST FOR PROPOSALS Sheboygan County Health and Human Services Department Division of Community Programs Food Preparation for Senior Dining Sites The Sheboygan County Health and Human Services Department–Division of Community Programs–Elder Services operates several Senior Dining sites throughout Sheboygan County. The program is designed to provide food and fellowship to the Senior Citizens of the County. This program is the result of state and federal initiatives aimed at providing the elderly with the opportunity for nutritious meals at a reasonable cost. As such, the program is made possible through both State and Federal Grants. Sheboygan County is now accepting proposals for food preparation in a centralized location to supply these sites. Proposals, including forms provided in the specification packet, must be on file with the Purchasing Agent no later than 3:00 PM local time at the location specified within the bid documents on December 1st, 2025 in order to be considered. All proposals received become the property of Sheboygan County and shall remain in effect for not less than sixty days beyond the due date. This contract is subject to the requirements of Title 49, Code of Federal Regulations, Part 26, Participation by Disadvantaged or Woman Owned Business Enterprises in Department of Human Services Financial Assistance Programs. The contractor shall not discriminate on the basis of race, color, national origin, or sex in the performance of this contract. The contractor shall carry out applicable requirements of 49 CFR Part 26 in the award and administration of this DOT-assisted contract. Failure by the contractor to carry out these requirements is a material breach of this contract, which may result in the termination of this contract or such other remedy as County deems appropriate. Each subcontract the contractor signs with a subcontractor must include the assurance in this paragraph (see 49 CFR 26.13(b)). Sheboygan County reserves the right to reject any proposals, cancel this solicitation, waive any informality in the proposal process, and award the bid deemed most beneficial to Sheboygan County.
