The State of Indiana to the Defendant(s) above named, and any other person who may be concerned.
You are hereby notified that you have been sued in the Court above named.
The nature of the suit against you is:
Complaint on Note and to Foreclose Mortgage on Real Estate Against the property commonly known as 4712 State Road 121, Richmond, IN 47374-8224 and described as follows:
A part of the Southeast Quarter of Section Twenty-six (26), Township Fourteen (14), Range One (1) West, bounded and described as follow, to-wit: 
Commencing at a point in the north line of said quarter section, which point is one thousand five hundred seventy-one (1.571) feet east of the northwest corner of said Quarter Section: thence from said beginning point west along the north line of said quarter three hundred eighteen (318) feet to the northeast corner of a tract conveyed to Bert A. Baker, et ux. and recorded in Deed Record 206, at page 532: thence soothe along Baker's east line seven hundred thirty-four (734) feet, more or less, to the center line of the New Paris Pike" thence north seventy-seven (77) degrees thirty (30) minutes east, along the center line of said New Paris Pike, four hundred twenty-one (421) feet; thence northwardly six hundred twenty-two and fifty-eight hundredths (622.58) feet, more or less, to the north line of said quarter section and the place of beginning containing an area of six (6) acres, more or less.
 
ALSO, A part of the Southeast Quarter of Section Twenty-six (26), Township Fourteen (14), Range One (1) bounded and described as follows, to-wit: 
Beginning at a point in the centerline of State Route #121 (New Paris Pike) which point is located as follows: Commencing at a point in the west line of said quarter section, where the center line of said State Route #121 intersects the same; thence along the centerline north seventy-six (76) degrees thirty (30) minutes east, five hundred sixteen (516) feet: thence along said center line north seventy-seven (77) degrees thirty (30) minutes east, one thousand one hundred seventy-five (1,175) feet to the place of beginning: thence from said beginning point running in a northerly direction six hundred twenty-two and fifty-eight hundreds (622.58) feet, more or less, to a point in the north line of said quarter section, one thousand five hundred seventy-one (1,571) feet east of the northwest corner of said quarter; thence east along the north line of said quarter to the westerly right of way line of US Intestate Highway #70; thence in a southerly direction along said westerly right of way line following calls: south thirty-nine (39) degrees thirteen (13) minutes east ninety-eight and two tenths (98.2) feet; thence south fifty-three (53) degrees fifteen (15) minutes east, one hundred three and one-tenth (103.1) feet; thence south thirty-nine (39) degrees thirteen (13) minutes east, three hundred (300) feet: thence south thirty-three (33) degrees thirty (30) minutes east, one hundred seventy (170) feet more or less to the centerline of State Route #121; thence in a southwesterly direction along the centerline of said State Route #121 four hundred twenty-five (425) feet, more or less to the place of beginning.
 
Excepting: A part of the Southeast Quarter of Section 26, Township 14, North, Range 1 West, in Wayne Township, Wayne County, Indiana, and being more particularly described as follows.
Commencing at the northwest corner of said Southeast Quarter; thence south 89 degrees 26 minutes 27.4 seconds east 1571.00 feet along the north line of said quarter to a point; thence south 07 degrees 40 minutes 54.1 seconds east 392.28 feet to an iron rod and the place of beginning of this description; thence south 07 degrees 40 minutes 54.1 seconds east 392.28 feet to an iron rod and the place of beginning of this description; thence north 79 degrees 17 minutes 30 seconds east 3393.89 feet to an iron rod at the west right-of-way of I.R. 70 per Project No. 70-5(17) 148; thence south 39 degrees 13 minutes east 62.62 feet along said right-of-way to a point; thence south 33 degrees 33 minutes 44.1 seconds east 101.49 feet along said right-of-way to a point; thence south 33 degrees 28 minutes 39.3 seconds east 74.43 feet along said of right-of-way to a railroad spike in the centerline of State Road 121; thence south 77 degrees 56 minutes 32 seconds west, 89.63 feet to an iron rod at the centerline intersection of State Road 121 and Gravel Pit Road: thence south 79 degrees 17 minutes 30 seconds west 360.00 feet along the centerline of State Road 121 to a railroad spike; thence north 07 degrees 40 minutes 54.1 seconds west 219.60 feet to the place of beginning containing an area of 2.000 acres, more or less.
 
Being a part of the Southeast Quarter of Section 26 Township 14 North Range 1 West in Wayne Township Wayne County Indiana and being more particularly described as follows:
Beginning at a point on the centerline of Indiana State Route 121 at the southwest corner of a tract of land described In Deed Record 206, page 532 in the records of the Wayne County Indiana Recorder and being Bert A. Baker's original southwest corner, said point also being nine hundred forty (940) feet south four hundred eighty-five and nineteen hundredths (485.19) feet north seventy-seven (77) degrees east, four hundred forty-two and eighty-one hundredths (442.81) feet north seventy-nine (79) degrees and thirteen (13) minutes east of the northwest corner of the Southeast Quarter of Section 26 and running thence from said beginning point, north two (2) degrees and fifty-six (56) minutes west, through a fence post, and along an established line two hundred ninety (290) feet to an existing concrete monument: thence north zero (0) degrees and fifty-one (51) minutes east, along an established fence line, four hundred sixty-one and twenty-nine hundredths (461.29) feet to a point on the north line
of Southeast Quarter: thence north eighty-nine (89) degrees and forty-nine (49) minutes east, along said north line, three hundred thirty-five and sixty hundredths (335.60) feet; thence south zero (0) degrees and forty-two (42) minutes east and along an established fence line six hundred eighty-seven and ninety-nine hundredths (687.99) feet to a point on the centerline of Indiana State Route 121; thence south seventy-nine (79) degrees and thirteen (13) minutes west, along said centerline, three hundred forty-two (342) feet to the place of beginning, containing an area of five and sixty-five hundredths (5.65) acres. more or less.
 
EXCEPTING THEREFROM, the following described real estate: A part of the Southeast Quarter of Section 26 Township 14 North Range 1 West, and bounded as follows to-wit:
Commencing at a point in the center of State Highway No. 121 seventy-seven (77) feet east of Bert A. Baker's southwest corner running thence northwardly two hundred seventy-three (272) feet to a stake said stake being the northeast corner of Robert Lee Baker's land; thence eastwardly sixty-two (62) feet to a stake being two hundred sixty-nine (269) feet north of the center of said State Highway No. 121; thence south parallel with said grantee's west line two hundred sixty-nine (269) feet to the center of said State Highway No. 121; thence Westwardly along the center of said State Highway to the place of beginning, heretofore conveyed to John C. Goeke and Lola Goeke, husband and wife, on September 4, 1948 and recorded in Deed Record 227, page 4 of the records of the Recorder of Wayne County, Indiana.
 
ALSO EXCEPTING THEREFROM, the following described real estate: A part of Southeast Quarter of section 26, Township 14 North, Range 1 West, and bounded as follows, to-wit: Commencing on the west line of Bert A. Baker's land two hundred ninety (290) feet north of Bert A. Baker's southwest corner which corner is located in the center of State Highway No. 121; thence southwardly along said Bert A. Baker's west line two hundred ninety (290) feet to the center of State Highway No.121; thence eastwardly along the center of State Highway No. 121, seventy-seven (77) feet; thence northwardly two hundred seventy-three (273) feet to a stake; thence northwestwardly seventy-seven (77) feet to the place of beginning heretofore conveyed to Robert Lee Baker, unmarried, on September 4, 1948 and recorded in Deed Record 239, page 107 of the records of the Recorder of Wayne County, Indiana.
 
ALSO EXCEPTING THEREFROM, the following described real estate: Situated in the Southeast Quarter of Section Twenty-six (26), Township Fourteen (14) North, Range One (1) West, Wayne Township, Wayne County, Indiana, being part of a 5.65 acre tract descried in Deed Records Volume 360 page 199, in the office of the Wayne County Recorder, and being more particularly described as follows: 
Beginning at the southeast corner of a 0.38 acre tract on the centerline of State Route 121 as described in Deed Records Volume 440, Page 453, said corner being more particularly described by records dimension as follows: 
Commencing at the northwest corner of said Southeast Quarter, thence South 940 feet; thence North 77 degrees East 485.19 feet; thence North 79 degrees 13 minutes East 581.81 feet to the TRUE POINT OF BEGINNING for the tract herein described, witness an iron pin found North 02 degrees 45 minutes 00 seconds West 30.00 feet; thence North 02 degrees 45 minutes and 00 seconds West 269.27 feet, along the east line said 0.38 acre tract, to a iron pipe found; thence North 82 degrees 41 minutes 00 seconds East 45.00 feet, along a new division line to an iron pin set; thence South 02 degrees 45 minutes 00 seconds East 266.52 feet along a new division line, to a point witness an iron pin set North 02 degrees 45 minutes 00 seconds West 30.00 feet; thence South 79 degrees 13 minutes 00 seconds West 45.30 feet along the centerline of the State Route 121, to the point of beginning, containing 0.276 acres, more or less.

If you have a claim for relief against the plaintiff arising from the same transaction or occurrence, you must assert it in your written answer or response.
You must answer the Complaint in writing, by you or your attorney, within thirty (30) days after the Third Notice of Suit, and if you fail to do so a judgment by default may be entered against you for the relief demanded, by the Plaintiff.
