A Petition having been filed in this Court alleging that the above-named child in the care of the Westchester County Department of Social Services should be committed to the guardianship and custody of the Westchester County Department of Social Services.
YOU ARE HEREBY SUMMONED to appear before this Court FOR AN APPEARANCE at 131 Warburton Ave, Yonkers, NY 10701 on the 28TH day of December 18, 2025 at 02:00 PM and on January 14, 2026 at 10:00 AM of said day, to show cause why the Court should not enter an order committing the guardianship and custody of the child to the Petitioner, Westchester County Department of Social Services, as provided by law.
PLEASE TAKE NOTICE, that if guardianship and custody of the child is committed to the Westchester County Department of Social Services, the child may be adopted with the consent of the Westchester County Department of Social Services, without your consent or further notice to you.
PLEASE TAKE FURTHER NOTICE, that you have the right to be represented by a lawyer, and if the Court finds you are unable to pay for a lawyer, you have the right to have a lawyer assigned by the Court.
PLEASE TAKE FURTHER NOTICE, that if you fail to appear at the time and place noted above, or at any adjourn date, the Court will hear and determine the petition upon inquest as provided by law and may, after hearing, terminate your parental rights and commit the guardianship and custody of the subject child to the Petitioner.
