REQUEST FOR CLAIMS AGAINST SOUTHERN BENEFIT CONSULTANTS, LLC, A GEORGIA LIMITED LIABILITY COMPANY   Notice is given that a Statement of Commencement of Winding Up of Southern Benefit Consultants, LLC, a Georgia limited liability company with its registered office at 3633 Wheeler Road Suite 230, Augusta, Richmond County, Georgia 30909 has been delivered to the Secretary of State of Georgia in accordance with the Georgia Limited Liability Company Act. Southern Benefit Consultants, LLC requests that any person with a claim against it provide the name of the claimant, the address of the claimant, and a written description of the nature and amount of the claim to Donsbach Lewis LLC, P.O. Box 212139, Martinez, GA 30917-2139. Except for claims that are contingent at the time of the filing of the Statement of Commencement of Winding Up, or that arise after the filing of the Statement of Commencement of Winding Up, a claim against Southern Benefit Consultants, LLC not otherwise barred will be barred unless a proceeding to enforce the claim is commenced within two years after the publication of this notice. Claims not otherwise barred that are contingent at the time of the filing of the Statement of Commencement of Winding Up, or that arise after the filing of the Statement of Commencement of Winding Up, will be barred unless a proceeding to enforce the claim in commenced within five years after the second publication of this notice or two years after the filing of a Certificate of Termination by Southern Benefit Consultants, whichever is later.
