TO: KAYLA CORNELIUSEN, parents and/or guardians of the above-named child.
1. The child, LILLY REYES, has filed a Motion for Termination of Parent-Child Relationship pursuant to Title 8 of the Arizona Revised Statutes and Rule 351 of the Arizona Rules of Procedure for the Juvenile Court.
2. The Court has set an Initial Severance Hearing on March 3, 2026, at 11:30 a.m., and a Publication Hearing on April 9, 2026, at 11:00 a.m., at the Maricopa County Superior Court, 3131 W. Durango Street, Phoenix, Arizona 85009, call-in number (917) 781-4590, Teams Code: 287-752-075 #, Teams Link: https://tinyurl.com/jbazmc-JUJ02, before the Honorable Justin Beresky, for the purpose of determining whether any parent or guardian named herein is contesting the allegations in the motion.
3. You and your child are entitled to have an attorney present at the hearing. You may hire your own attorney or, if you cannot afford an attorney and want to be represented by an attorney, one may be appointed by the Court.
4. You have a right to appear as a party in this proceeding. You are advised that your failure to personally appear in court at the initial hearing, pretrial conference, settlement conference, status conference or termination adjudication, without good cause shown, may result in a finding that you have waived your legal rights and have admitted the allegations in the Motion. In addition, if you fail to appear, without good cause shown, the hearing may go forward in your absence and may result in the termination of your parental rights based upon the record and the evidence presented to the Court.
