NOTICE TO CREDITORS, HEIRS, AND BENEFICIARIES OF HELEN MAXINE BURKS, DECEASED, AND THE HELEN MAXINE BURKS REVOCABLE TRUST In The Matter Of: The Helen Maxine Burks Revocable Trust U/A/D December 7, 2004, as amended and restated To all persons regarding Helen Maxine Burks, deceased, who died on or about the 8th day of February, 2026. You are hereby notified that Brenda Lee Burks, 3806 Lindalavista Way, Des Moines, IA 50310, is serving as Trustee of the Helen Maxine Burks Revocable Trust under agreement dated December 7, 2004, as amended on April 20, 2021, and as amended on January 11, 2022 ("the Trust"). At this time, no probate administration is contemplated with regard to the estate of Helen Maxine Burks, deceased. Any action to contest the validity of the Trust must be brought in the District Court of Polk County, Iowa, within the later to occur of four months from the date of the second publication of this notice, or thirty (30) days from the date of mailing this notice to all heirs of the decedent, spouse of the decedent and qualified beneficiaries under the Trust whose identities are reasonably ascertainable. Any claim or suit not filed within this period shall be forever barred. Notice is further given that all persons indebted to the decedent or to the Trust are requested to make immediate payment to the undersigned Trustee.
