NOTICE OF REQUEST FOR PROPOSALS FOR
THIRD PARTY BILLING SERVICE FOR MUNICIPAL AMBULANCE SERVICES
Notice is hereby given that sealed Proposals will be received by the Borough of Fair Lawn (“Borough”), 8-01 Fair Lawn Avenue, Fair Lawn, New Jersey 07410, at the Municipal Building, Council Chambers, Tuesday, June 23, 2026 at 10:00 A.M. prevailing time for the following contract:
Third Party Billing Service for Municipal Ambulance Services
Proposals shall be opened on Tuesday, June 23, 2026 at 10:00 A.M. prevailing time at the above location. Proposals must be submitted by the foregoing date and time on the Proposal Forms furnished in the Request for Proposals (RFP) and delivered in sealed envelopes clearly marked “Third Party Billing Service for Municipal Ambulance Services.” Faxed, electronic or emailed Proposals will not be accepted.
Proposers must include their name and address on the outside of the envelope. Proposals prepared on forms other than those provided by the Borough may be rejected. Proposals received after the date and time set forth herein, regardless of cause of delay, will not be accepted but will be returned unopened.
The RFP, including the Proposal Forms and Scope of Services, may be obtained by contacting the Borough Clerk’s Office, telephone 201-794-5340, between the hours of 8:30 a.m. and 4:30 p.m., Monday through Friday, excluding legal holidays or by emailing bids@fairlawn.org to pick-up or discuss arrangements for the delivery of the RFP documents. The Borough Clerk’s Office will insure that the electronic RFP documents are distributed; however, the Borough will not be responsible if an interested person(s) or firm(s) cannot download the RFP documents. The Borough only assumes responsibility for mailing the RFP documents to the interested person(s) or firm(s) and not for proper and timely delivery thereof to such interested person(s) or firm(s).
A certified check, cashier’s check or bid bond in the amount of ten percent (10%) of the Proposal amount, but not in excess of Twenty Thousand Dollars ($20,000), must be submitted with the Proposal, guaranteeing that if a contract is awarded, the Proposer shall execute the contract and deliver the bond(s), if any, required by the RFP documents. The Proposal Bond shall be obtained from a surety company that is authorized to do business in the State of New Jersey. Such Proposal Bond shall not contain any conditions to the obligations of the surety company issuing the Proposal Bond. Proposal Bonds signed by an Attorney-in-Fact shall be accompanied by an executed and certified Power-of-Attorney.
The Borough reserves the right to consider the Proposals for sixty (60) days after the receipt thereof, and further reserves the right to reject any or all Proposals, to abandon the procurement process, to waive any informality and make such awards or take such action as may be in the best interest of the Borough, all in accordance with applicable law. Any award of a contract hereunder shall be made in accordance with the competitive contracting process, N.J.S.A. 40A:11-4.1 et seq.
The Successful Proposer shall be required to comply with the requirements of N.J.S.A. 10:5-31 et. seq., N.J.A.C. 17:27 (Affirmative Action), P.L. 1963. C150, and Americans with Disabilities Act of 1990 (42 U.S.C. 12101, et. seq.), and N.J.S.A. 52:32-44 (Business Registration) and shall submit proof of the same.
