12398737 NOTICE to BIOLOGICAL MOTHER of BABY “SAFE HAVEN GIRL” COFFEE: The Department of Children’s Services has filed a petition seeking to declare Baby “Safe Haven Girl” Coffee an abandoned child, to terminate your parental rights, and to free this child for adoption. If you fail to seek contact with the child through the Department of Children’s Services or to revoke the voluntary delivery within thirty (30) days of the last date of publication, your failure shall constitute abandonment of the child and of your interests and shall constitute grounds for termination of your parental rights.
