Columbia Orthopaedic Group Legal Notice
Notice of Data Incident. On April 19, 2026, after discovering suspicious activity on a portion of its computer network, Columbia Orthopaedic Group promptly initiated an internal investigation and took steps to contain the incident. Columbia Orthopaedic Group also engaged a forensic security firm to assist with its investigation and to confirm the security of its computer systems. The forensic investigation determined that a third party accessed Columbia Orthopaedic Group's computer systems beginning around April 16, 2026, through April 19, 2026, and may have acquired certain files from its systems during that time. Columbia Orthopaedic Group has determined that the files involved contain personal information related to certain patients. The information varies among individuals but may include names, dates of birth, addresses, health insurance information, Social Security numbers, driver's license numbers, or medical information. Columbia Orthopaedic Group has begun to send written notifications to the individuals whose information may have been in the files. Individuals should refer to those notices regarding steps they can take to protect themselves, should they feel the need to do so. As a precautionary measure, individuals may review their account statements and monitor credit reports for any suspicious activity on an account. They should promptly notify the financial institution or company with which the account is maintained if appropriate. They should also promptly report any fraudulent activity or any suspected incidents of identity theft to proper law enforcement authorities, including the police and their state's attorney general. Individuals may also wish to review the tips provided by the Federal Trade Commission ("FTC") on fraud alerts, security/credit freezes and steps that they can take to avoid identity theft.
