NOTICE OF ADOPTION STATE OF INDIANA ) MARION COUNTY SUPERIOR COURT 22 )SS: COUNTY OF MARION ) CAUSE NO: 49D22-2602-AD-008151 IN RE THE ADOPTION OF: ) ) WHITE, Rashawn Jeffrey, Jr., and ) ) BROWN, Juanita Shawnea' ) ) MINOR CHILDREN ) NOTICE OF ADOPTION Rashawn Jobaa White, is notified that a petition for adoption of a child, named Rashawn Jeffrey White, Jr., born to Samantha Shanea Brown on August 26, 2015, was filed in the office of the clerk of Marion County Superior Court, 675 Justice Way, Indianapolis, Indiana 46203 and 317-327-4747. The petition for adoption alleges that the consent to adoption of Rashawn Jobaa White is not required pursuant I.C. 31-19-9-8-(a)(1), (2), (3) and (11). If Rashawn Jobaa White seeks to contest the adoption of the child, Rashawn Jobaa White must file a motion to contest the adoption in accordance with IC 31-19-10-1 in the above named court not later than fifteen (15) days after the date of service of this notice. If Rashawn Jobaa White does not file a motion to contest the adoption within fifteen (15) days after service of this notice, the above-named court will hear and determine the petition for adoption. The consent to adoption of Rashawn Jobaa White will be irrevocably implied and Rashawn Jobaa White will lose the right to contest either the adoption or the validity of Rashawn Jobaa White's implied consent to the adoption. No statement made to Rashawn Jobaa White relieves Rashawn Jobaa White of Rashawn Jobaa White's obligations under this notice. This notice complies with IC 31-19-4.5-3 but does not exhaustively set forth a person's legal obligations under the Indiana adoption statues. A person being served with this notice should consult the Indiana adoption statutes. Rashawn Jobaa White, who has been named the father of the child born to Samantha Shanea Brown on November 21, 2016, or who claims to be the father of the child born to Samantha Shanea Brown on November 21, 2016, is notified that a petition for adoption of the child was filed in the office of the clerk of Marion County Superior Court, 675 Justice Way, Indianapolis, Indiana 46203 and 317-327-4747. If Rashawn Jobaa White seeks to contest the adoption of the child, he must file a motion to contest the adoption in accordance with IC 31-19-10-1 in the above-named court not later than fifteen (15) days after the date of service of this notice. If Rashawn Jobaa White does not file a motion to contest the adoption within fifteen (15) days after service of this notice, the above-named court will hear and determine the petition for adoption. His consent will be irrevocably implied, and he will lose his right to contest either the adoption or the validity of his implied consent to the adoption. He will lose his right to establish his paternity of the child, by affidavit or otherwise, in Indiana or any other jurisdiction. Nothing Samantha Shanea Brown or anyone else says to Rashawn Jobaa White relieves Rashawn Jobaa White of his obligations under this notice. Under Indiana law, a putative father is a person who is named as or claims that he may be the father of a child born out of wedlock but who has not yet been legally proven to be the child's father. For purposes of this notice, Rashawn Jobaa White is a putative father under the laws in Indiana regarding adoption. This notice complies with IC 31-19-4-5 but does not exhaustively set forth a putative father's legal obligations under the Indiana adoption statutes.
