NOTICE OF DISSOLUTION OF THERAPY GM, LLC NOTICE OF DISSOLUTION OF THERAPY GM, LLC IS HEREBY GIVEN in accordance with the provisions of Wisconsin Statutes Chapter 183 that: (1) Therapy GM, LLC, a Wisconsin limited liability company, has been dissolved and terminated effective July 21, 2026, (2) all persons with claims, whether known or unknown, against Therapy GM, LLC or its managers or members, in their capacities as such, must present such claims in writing via first-class mail, postage prepaid, to Therapy GM, LLC, at 114 Arundel Drive, Archer Lodge, NC 27527, with a copy to Attorney Caitlin E. Riordan at McCarty Law LLP, 2401 East Enterprise Avenue, Appleton, WI 54913; (3) each claim must describe in detail the nature and amount of the claim or an estimate of such amount, the date upon which the claim arose, and the facts and circumstances giving rise to the claim. Each claim must be accompanied by documentation supporting or substantiating the claim; and (4) any alleged claims against Therapy GM, LLC, or its managers or members will be barred unless a proceeding to enforce the claim is commenced within 2 years after the publication of this notice.
