NOTICE OF DISSOLUTION of SY-CARE LLC. NOTICE OF DISSOLUTION of SY-CARE LLC., (ALSO KNOWN AS SYCARE LLC.) Notice is hereby given that SY-CARE LLC a limited liability company organized under the laws of the State of Colorado, has been dissolved effective October 22, 2025. All persons having claims against SY-CARE LLC are required to present their claims in accordance with this notice. Claims must include the name and address of the claimant, the amount of the claim, and the basis for the claim. Claims should be sent to: SY-CARE LLC, Attn: Jason Chippeaux, 41 Montebello Road, Suite 200, Pueblo, CO 81001; and a duplicate notice should be mailed to Valley-Wide Health Systems, Inc., Attn: Legal Department, 128 Market Street, Alamosa, CO 81101. Pursuant to section 7-90-912 of the Colorado Revised Statutes, unless sooner barred by any other statute limiting actions, any claim against the dissolved entity will be barred unless an action to enforce the claim is commenced within five years after the publication of this notice or within four months after the claim arises, whichever is later.
