NOTICE TO CREDITORS AND CLAIMANTS Leaf Mental Health, Inc., a Delaware corporation (the "Corporation"), has elected to dissolve and is in the process of winding up its affairs pursuant to Section 275 and the long-form dissolution procedures of Sections 280–282 of the Delaware General Corporation Law. All persons having claims against the Corporation must present them in writing to: Leaf Mental Health, Inc. Attn: Dissolution Claims PO Box 6934, Ellicott City, MD 21042 Each claim must state the amount claimed, the basis of the claim, and the claimant's contact information. Supporting documentation must be included. Claims must be received no later than February 9, 2026. Claims not received by this date will be barred. The Corporation may reject, reduce, or object to any claim in whole or in part. If your claim is rejected, you will be notified and will have the rights described in Section 280 of the Delaware General Corporation Law.
