NOTICE TO CREDITORS OF DENNIS W. AND KAREN H. CHASE TRUST, dated May 17, 2013 Gerald L. Cox, whose address is c/o ProvenLaw, PLLC, 216 W. St. George Blvd., Ste. 200 St. George, UT 84770, is the Trustee of the DENNIS W. AND KAREN H. CHASE TRUST, dated May 17, 2013 (the "Trust"). Karen Howe Chase, Grantor of the Trust, died on August 18, 2025, and Dennis Whitaker Chase, Grantor of the Trust, died on January 8, 2026. Utah Code Ann. § 75B-2-508 and 75-3-801, provide that any creditor having a claim against the Trust or the Estate of Karen Howe Chase and Dennis Whitaker Chase must file their claim with Gerald L. Cox, Trustee, or Kelsey J. Haslam, attorney for Gerald L. Cox, Trustee, at 216 W.
