NOTICE TO INTERESTED PERSONS NOTICE IS HEREBY GIVEN that Janice M. Acorn has been appointed and has qualified as the Trustee of the Forum Family Trust, created on March 29, 2007, and is currently in existence. All persons having claims against Rhea E. Forum, decedent, or the Forum Family Trust are required to present their claims, with proper vouchers, within four months after the date of first publication of this notice, as stated below, to the Trustee at 188 West B Street, Bldg. N, Springfield, Oregon 97477, or the claims may be barred.
