Notice of Activity and Use Limitation SITE NAME: Residences on Madison Street SITE ADDRESS: 5 Madison Street, Worcester RELEASE TRACKING NUMBERS: 2-50986 & 2-51471 A release of oil and/or hazardous materials has occurred at this location, which is a disposal site as defined by M.G.L. c. 21E, § 2 and the Massachusetts Contingency Plan, 310 CMR 40.0000. On October 21, 2025,SMC Residences on Madison, Limited Partnership recorded with the Worcester South District Registry of Deeds a Notice of Activity and Use Limitation (AUL) on the disposal sites, pursuant to 310 CMR 40.1070 through 40.1080. The AUL will limit the following site activities and uses on the above property: (i) residential uses without the evaluation of a vapor intrusion pathway; (ii) constructing new habitable structures in the portion of the property subject to the AUL; and (iii) conducting other activities that in the opinion of a Licensed Site Professional are inconsistent with maintaining a condition of no significant risk.
