Notice to Creditors The Ralph C. And Geraldine E. Collins Family Trust dated August 26, 2008 NOTICE IS GIVEN to all creditors of the Decedent that: 1. The original Grantors were Ralph C. Collins and Geraldine E. Collins. The Grantors are now deceased. The Trust is irrevocable. 2. Jeffrey C. Collins is the Successor Trustee of the Trust. 2. All persons having claims against the Trust, Decedent or the Decedent's Estate are required to present their claim to the Trustee within four (4) months after the date of the first publication of this Notice or be forever barred. 3.
