Notice to Creditors The Smith Family Trust, dated July 1, 1991 NOTICE IS GIVEN to all creditors of the Decedent that: 1. The original Grantors were Michael J. Smith and Mary W. Smith. The Grantors are now deceased. The trust is irrevocable. 2. Martin C. Smith is the Successor Trustee of the Trust. 2. All persons having claims against the Trust, Decedent or the Decedent's Estate are required to present their claims to the Trustee within four (4) months after the date of the first publication of this Notice or be forever barred. 3.
