PUBLIC NOTICE OF HOSPICE CLOSURE Notice is hereby given that Grace Hospice of Wisconsin, dba Crossbridge Hospice, located at 2514 S. 102nd Street, Suite 276, West Allis, WI 53227, will voluntarily cease hospice operations effective February 3, 2026 and will no longer participate in the Medicare program under Title XVIII of the Social Security Act. All hospice services will end as of the effective date. All patients will be discharged or transferred prior to closure, and no further care or services will be provided after February 3, 2026. Medical records will be maintained in accordance with state and federal requirements.
