Public Notice This serves as formal notice pursuant to Florida Administrative Code 64B8-10.002(4) and F.S. 456.058 that Mason Long, M.D. formally announces his retirement from his medical practice currently known as Mason H Long, MD, PA located at Suite 325 , 301 Health Park BLVD, Saint Augustine, FL 32086 on October 5th, 2025. UF Health Primary Care-Whetstone has kindly agreed to securely house and manage the medical records. Any current patient of Dr Long's can become a patient of the UF Health Primary Care by registering in person or on-line (stjohns.ufhealth.org). Alternatively, patients may chose a new primary care provider anywhere, and UF Health can transfer medical records with completion of signed forms . all of Dr Long's patient medical records will remain securely within the UF Health Primary Care-Whetstone electronic medical records system.
