Public Notice Participants in the JM Family Automotive LLC Health plan who have received health care at the JM Family Port Health and Wellness Center. Please take notice that JM Family Automotive LLC (JM Family) is closing the Port Health and Wellness center and relocating to the Blount Island Health and Wellness Center. Those patients who have the proper security clearance may still receive health care at the Blount Island H&WC at 8975 Dave Rawls Boulevard, Jacksonville, Florida 32226, and all others at the Westlake Health & Wellness Center located at 9985 Pritchard Road, Jacksonville, FL 32219. Your health care records from the Port Health and Wellness Center are available at the Blount Island and Westlake Health and Wellness Centers. Dr. Richard Valenzuela, who provides care to JM Family plan participants at the Port Health & Wellness Center, will continue to treat JM Family patients, who have the proper security clearance, at the Blount Island Health and Wellness Center.
