Request for Public Comment The State of Wisconsin Department of Health Services (DHS) is seeking public comment on its draft § 1915 (c) Home and Community-Based Services (HCBS) Waiver renewal application for the Children's Long-Term Support (CLTS) Waiver Program, which includes a § 1915(b)(4) Waiver Fee-for-Service Selective Contracting Program application. The CLTS Waiver Program provides community-based supports and services from birth through age 21 for children with long-term support needs. The draft waiver renewal application is available on the DHS website at https://www.dhs.wisconsin.gov/clts/renewal.htm.
