This notice is being provided pursuant to New York Business Corporation Law §1007 to notify any claimants and to provide information about the availability of patient medical records. Notice to Creditors and Claimants All persons or entities with claims against the Corporation must present them in writing to the address below. Each claim must include the claimant's name, address, telephone number, the amount of the claim, and the basis for the claim. Claims must be received no later than two months from this notice.
