A copy of the Notice is available at www.elicense.ohio.gov. The Notice states that the State Medical Board of Ohio intends to determine whether or not to take disciplinary action against his license to practice medicine and surgery in Ohio due to providing false or misleading information during the application or renewal process. Dr. Zewe is entitled to a hearing in this matter if such hearing is requested within thirty (30) days of the publication of this notice. Dr. Zewe may appear at such hearing in person, by his attorney, or by such other representative permitted to practice before this agency, or he may present his position, arguments or contentions in writing.
